Dr. Therese Lucietto-Sieradzki, M.D.
Dr. Therese Lucietto-Sieradzki graduated from Northwestern University in Evanston, IL, in 1989 with a Bachelor of Science in Materials Science Engineering. After five years in the industry, she pursued her passion for medicine and attended and graduated from Northwestern University Feinberg School of Medicine in Chicago, IL. Her focus is on family medicine, and she completed her post-graduate training at St. Francis Hospital and Ravenswood-Illinois Masonic Hospital in the Chicago area.
In 2001, Dr. Lucietto moved to Belvidere to care for under-served patients as a board-certified family physician. In 2005, she was inspired by her experiences to open her own medical practice, Your Family Doctor, to provide the high-quality, personalized care she believes every patient deserves.
For more than 20 years, Dr. Lucietto has practiced medicine in Northern Illinois and Southern Wisconsin, and she has medical licenses in Illinois, Wisconsin, Louisiana, and Guam. In 2015, Dr. Lucietto became certified in homeopathic medicine, and she remains dedicated to lifelong learning for the betterment of her patients as a family doctor.
Dr. Lucietto is committed to providing the proper amount of health services and time to address her patients' concerns. She never stops looking for solutions. If she is unable to help, she gets patients access to a specialist who can. She has privileges at three different hospitals: Swedish American Hospital, OSF Saint Anthony Medical Center, and Beloit Memorial Hospital. She also works part-time at Rosecrance, a residential substance abuse treatment center in Rockford, IL. Dr. Lucietto volunteers as the ring side physician for the Patriots Boxing Club run by Jimmie Goodman in Rockford, IL.
Sarah B. Lyons, Certified Nurse Practitioner
Our caring team of professionals includes Sarah B. Lyons, APN, FNP-BC, a board-certified Family Nurse Practitioner. She joined Your Family Doctor in August of 2017 and came to us with 13 years of experience in health care services. After her first four years as a professional dental hygienist, Sarah pursued further education and provided patient care as an emergency room nurse for nine more years. After spending four years in a trauma center on the west side of Chicago, she moved to a community hospital in the western suburbs.
Sarah returned to school again and received her post-master's degree from Northern Illinois University which brought her to Your Family Doctor. Sarah enjoys treating patients of all ages and providing a focus on preventive medicine. She treats many chronic conditions. She currently works two days a week in our health care clinic and spends the remainder of her time at home with her family raising her three daughters.
Family nurse practitioners are licensed, board-certified clinicians who have obtained an advanced degree in nursing that includes additional classroom work and clinical experience in health services. They blend their nursing background with advanced education to provide medical treatment. They diagnose and treat chronic health conditions with an added focus on disease prevention and health maintenance. At Your Family Doctor, they work collaboratively with Dr. Lucietto to provide comprehensive treatment to patients and achieve the best possible outcomes.
